The Nurse Assessment: The RN Field Nurse Supervisor will meet you in your home or at your hospital bedside and will work with you to assess all of your safety, medical and social needs. The RN Field Nurse Supervisor will then develop an actionable and measurable plan of care that is specifically tailored to your loved one’s needs.
